Position Summary
We are seeking an experienced, patient-centered Adult Nurse Practitioner (ANP/AGNP) or Family Nurse Practitioner (FNP) licensed in North Carolina to provide telemedicine services on an independent contractor (1099) basis.
The Nurse Practitioner will provide synchronous and asynchronous telemedicine care to patients receiving HIV treatment, HIV pre-exposure prophylaxis (PrEP), Hepatitis C (HCV) evaluation and treatment, STI services, and related clinical care.
This is a flexible, PRN/as-needed contractor position designed to provide clinical coverage based on patient volume and organizational needs.
The ideal candidate is comfortable providing evidence-based care through a virtual healthcare model, has experience or a strong interest in HIV and sexual health, and provides compassionate, culturally responsive, LGBTQ+ affirming care.
The contractor must:
Hold a current, unrestricted North Carolina Registered Nurse (RN) license or applicable multistate privilege.
Hold current authorization/approval to practice as a Nurse Practitioner in North Carolina.
Maintain current national board certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult-Gerontology Nurse Practitioner (AGNP) appropriate to the patient population served.
Maintain a valid Collaborative Practice Agreement with a North Carolina-licensed supervising physician as required by North Carolina law and applicable regulatory requirements.
Ensure the collaborative practice arrangement appropriately encompasses the clinical services being provided, including telemedicine management of HIV, PrEP, Hepatitis C, STIs, and related clinical services.
Maintain documentation and other requirements associated with the collaborative practice arrangement and comply with applicable requirements of the North Carolina Board of Nursing and North Carolina Medical Board.
Immediately notify the organization of any lapse, termination, restriction, investigation affecting practice eligibility, or material change involving the contractor's collaborative practice arrangement, NP approval, RN license, national certification, prescribing authority, or other credential required to perform contracted services.
Maintain prescribing authority and DEA registration when required for services performed.
Maintain professional liability/malpractice insurance at the limits specified in the independent contractor agreement.
Maintain all required continuing education, certifications, registrations, and professional credentials necessary to perform the contracted services.
Maintaining all required North Carolina licensure, Nurse Practitioner approval, national certification, prescribing authority, and collaborative practice requirements is a condition of continued engagement as an independent contractor.
Minimum Qualifications
Graduate of an accredited Nurse Practitioner program.
Current, unrestricted North Carolina licensure and Nurse Practitioner approval/authorization.
Current national certification as an FNP, ANP, or AGNP appropriate to the population being treated.
Current qualifying collaborative practice arrangement as required for Nurse Practitioner practice in North Carolina.
Ability to provide telemedicine services in compliance with applicable state and federal requirements.
Strong clinical documentation and electronic medical record skills.
Strong written and verbal communication skills.
Ability to work effectively in a remote clinical environment with limited day-to-day supervision.
Reliable computer, high-speed internet connection, and a private environment appropriate for HIPAA-compliant telemedicine.
Preferred Qualifications
Prior experience providing HIV primary or specialty care.
Experience prescribing and managing HIV PrEP.
Experience evaluating and treating Hepatitis C.
Experience diagnosing and treating sexually transmitted infections.
Familiarity with current CDC, DHHS, AASLD/IDSA, and other applicable HIV, PrEP, STI, and HCV guidelines.
Experience providing synchronous and asynchronous telemedicine.
Experience serving LGBTQ+ communities and populations disproportionately affected by HIV, STIs, and viral hepatitis.
Familiarity with specialty pharmacy processes, medication access, prior authorizations, manufacturer assistance programs, and patient-assistance resources.
Schedule & Availability
This is a PRN/as-needed independent contractor position. Patient volume, hours, and scheduling may fluctuate based on organizational and patient-care needs.
Clinical services may include:
Scheduled telemedicine clinic coverage
PRN patient visits
Synchronous video encounters
Asynchronous clinical encounters
Laboratory and diagnostic result review
Prescription management
Patient messaging
Patient follow-up and care coordination
Specific availability, coverage expectations, and reasonable response-time requirements will be established by mutual agreement.
Compensation & Independent Contractor Status
This position is structured as an independent contractor (1099) engagement. Compensation will be established in the independent contractor agreement and may be based on completed encounters, scheduled clinical sessions, hourly availability, or another mutually agreed-upon methodology.
The contractor is responsible for applicable federal, state, and local taxes and for maintaining the licenses, certifications, insurance, collaborative practice arrangements, equipment, and other professional requirements assigned to the contractor under the independent contractor agreement.
The contractor relationship, including compensation, scheduling, responsibilities, professional expenses, termination provisions, and other terms, will be governed by a separate written Independent Contractor Agreement.
Nothing in this job description is intended to modify the legal relationship established by the parties' written agreement or to guarantee a specific number of patients, encounters, or hours.

Requirements
Primary Responsibilities
HIV Treatment
Conduct initial and follow-up telemedicine visits for patients living with HIV.
Review HIV viral loads, CD4 counts, resistance testing, STI screening, hepatitis testing, and other relevant laboratory results.
Initiate, continue, and manage antiretroviral therapy as clinically appropriate.
Assess medication adherence, tolerability, potential drug interactions, and barriers to treatment.
Identify patients requiring additional diagnostic evaluation, specialist consultation, or in-person care.
Provide patient education regarding HIV treatment, medication adherence, U=U (Undetectable = Untransmittable), transmission prevention, and long-term health maintenance.
HIV PrEP & Prevention
Evaluate patients for HIV PrEP eligibility and clinical appropriateness.
Initiate and manage oral and injectable PrEP therapies when clinically indicated.
Order and review HIV testing, renal function, STI screening, hepatitis testing, pregnancy testing when applicable, and other appropriate laboratory studies.
Provide counseling regarding HIV prevention, medication adherence, safer-sex practices, STI prevention, and recommended follow-up.
Evaluate patients for post-exposure prophylaxis (PEP) when clinically appropriate and transition eligible patients from PEP to PrEP.
Provide ongoing PrEP monitoring and medication management according to current clinical guidelines.
Hepatitis C
Evaluate patients with positive HCV screening tests or confirmed Hepatitis C infection.
Order and interpret appropriate confirmatory, pretreatment, and monitoring laboratory studies.
Evaluate patients for treatment eligibility and identify patients requiring specialist referral or additional evaluation.
Initiate and manage direct-acting antiviral therapy when clinically appropriate.
Monitor treatment response and appropriate post-treatment laboratory testing, including documentation of sustained virologic response.
Provide counseling regarding medication adherence, transmission, reinfection prevention, and follow-up care.
STI & Sexual Health Services
Order and interpret appropriate STI screening and diagnostic testing.
Diagnose and treat common sexually transmitted infections within the Nurse Practitioner's scope of practice.
Provide appropriate counseling regarding STI prevention, partner management, testing intervals, and risk reduction.
Integrate sexual health, HIV prevention, and viral hepatitis services into patient care when clinically appropriate.
Telemedicine Responsibilities
The Nurse Practitioner may provide care through both synchronous and asynchronous telemedicine modalities, as clinically appropriate and permitted by applicable laws, regulations, and professional standards.
Responsibilities may include:
Conducting scheduled and PRN synchronous video visits.
Reviewing and responding to asynchronous patient encounters.
Reviewing patient questionnaires, medical histories, laboratory results, medication requests, and other clinical information.
Reviewing normal and abnormal laboratory results and establishing appropriate follow-up plans.
Responding to clinically appropriate patient messages and care-team communications.
Ordering laboratory testing, diagnostic studies, medications, and referrals when clinically indicated.
Providing prescription renewals and ongoing medication management.
Coordinating care with pharmacies, laboratories, specialists, and other members of the healthcare team when necessary.
Determining when a patient's condition cannot be appropriately managed through telemedicine and facilitating referral to in-person, urgent, emergency, or specialty care.
Maintaining timely, accurate, and complete documentation in the electronic medical record.
Clinical & Professional Expectations
The contractor is expected to:
Practice in accordance with applicable federal and North Carolina laws, regulations, professional standards, and scope-of-practice requirements.
Follow current evidence-based HIV, PrEP, STI, and Hepatitis C treatment and prevention guidelines.
Exercise independent clinical judgment within the Nurse Practitioner's authorized scope of practice and collaborative practice arrangement.
Maintain patient confidentiality and comply with HIPAA and applicable privacy and security requirements.
Deliver compassionate, nonjudgmental, culturally responsive, and LGBTQ+ affirming care.
Maintain appropriate professional boundaries and standards of clinical documentation.
Participate in reasonable clinical quality-assurance, compliance, and peer-review activities when applicable.
Communicate significant clinical concerns to the appropriate members of the healthcare team.
Complete assigned encounters, documentation, prescriptions, laboratory review, and patient follow-up within established timeframes.
